Download the BIANC CBIS Application here. Complete/mail to BIANC, PO Box 10912, Raleigh, NC 27605
For a PDF copy of the BIANC CBIS application, click here for PDF.

Upcoming Classes: Call 919 618 3003 if you have a question about registering for a class. It you are past the deadline, you may still register for the class but may have to wait to take the exam which is on-line and can be proctored by a supervisor, a BIANC trainer, or another CBIS.

Greensboro hosted by Cone Health : Saturday June 2 and Saturday June 9, 2012 Deadline: May 15)

Asheville (Dates: June 14 and 15, 2012) Deadline: May 21

If you want to take a CBIS class from BIANC, please contact Shawn Chase or Susan Fewell at 919-833-9634 or call 919-618-3003. We will need an electronic version of your resume or CV sent to shawn.chase@bianc.net so that we can process your application and we send resume on to BIAA.

To register for a BIANC CBIS class, all forms and payment go to BIANC rather than to ACBIS

1. Please download and complete a BIANC CBIS application (PDF version of application)

2. Mail to BIANC with payment for the class. See Costs below. Checks should be made to BIANC.

Mail to: BIANC, PO Box 10912, Raleigh NC 27605

3. Email your resume or CV to shawn.chase@bianc.net

BIANC will then send you a confirmation, register you for the class and pay for your exam and will mail you a manual so you can study before the class.

The cost is $350 for the 2 day class, manual, exam, and lunch each day. If you would like to take the two day class and not sit for the exam, the cost is $150 and would include the class, manual and lunch each day. Some scholarships or reduced rates are available for survivors or family members to audit the class. Some organizations or professionals may prefer to study the manual on their own and just take the exam without the 2 days of training and we are happy to work with you on an individual basis. We can do a review and/or just proctor the exam for you. All checks are made to BIANC.

Please review the acbis.pro website to learn more about CBIS and to insure that you have the 500 hours of paid experience to qualify to sit for the exam. The CBIS and CBIST certifications are developed and administered through the Brain Injury Association of America and their requirement to sit for the exam is a high school education and 500 hours of paid work experience working with brain injury in the past 3 years. Personal and volunteer experience is not counted at this time.

** If you take our BIANC CBIS class, all forms and payments go to BIANC directly and we will register each person through ACBIS and BIAA and pay for your exam and send you a manual ahead of the class.

How can you become a proctor for someone?

BIANC can provide a proctor during the training period or will try to work with each applicant to find a proctor in their local community or a proctor from one of the BIANC Resouce Centers.. To be a proctor, you must either be a colleague of the CBIS applicant, the supervisor of the CBIS applicant or a CBIST or approved BIANC CBIS staff person. The process is simple.

Click on the link below and fill out the form. Where it says: "Name of Person or Organization for which you are serving as a group administrator/proctor". You can put "Brain Injury Association of NC: BIANC" and this will allow you to proctor other folks in the future or you can just put the name of the specific applicant that you are willing to proctor.

     GROUP ADMINISTRATOR/PROCTOR ACCOUNT SETUP APPLICATION.
The proctor will then receive an email which provides information that details the next steps of the testing process. To access the examination for CBIS, it will be necessry for the proctor to print off the "Exam Generation Card" with the applicant's "Registration number" and the UserId and Password that will be used to download each of the 12 sections of the exam. The proctor will also need the User Name and Password that BIANC used to track each applicant as these will be used to submit and sign each of the 12 sections of the CBIS exam. Shawn Chase or Susan Fewell will be happy to help you with this process. Call 919-833-9634 or 919-618-3003.
